DISCLOSURE AGREEMENT
CONFIDENTIALITY
All matters discussed during sessions are confidential and will not be disclosed without your written consent, except in the following circumstances as required by law:

If you give explicit permission to release information to a specific individual or agency

If there is suspected child abuse or neglect

If you present an imminent danger to yourself or others

If records are subpoenaed by a court of law
From time to time, I may consult with other licensed professionals to ensure high-quality care. In such cases, your identity will remain anonymous. Your signature below provides consent for such professional consultation.
FEES & PAYMENT
The standard fee is $250 per 60-minute session unless a discount, group rate, or promotional package has been applied. These discounts may be listed on my website or communicated directly.
Package rates must be paid in advance and generally offer a reduced per-session fee. Payment is due either before or at the time of service unless a written payment plan has been agreed upon.
CANCELLATION POLICY
Your scheduled time is reserved specifically for you. Cancellations made with less than 24hours’ notice will incur a $50 late cancellation fee. Emergencies and inclement weather may beconsidered exceptions, but timely communication is essential. Please arrive on time to ensure you receive your full session.
COMMUNICATION & ADMINISTRATIVE FEES

Brief phone calls (under 15 minutes) and emails are included at no cost

Calls exceeding 15 minutes may be billed at $1.25 per minute, prorated

Reports requested by medical professionals, legal representatives, or insurance providers will only be released with your written consent. A $25 administrative processing fee may apply
REFUNDS & RESULTS
There are no guarantees as to the outcomes of Wellness Counseling, Coaching, or Stress Management services, as results depend on individual participation and numerous personal actors. No refunds will be issued for services rendered. Your success depends equally on your commitment to the process. I pledge to support you to the best of my professional ability, and you are encouraged to participate with honesty, openness, and consistency.
